Background & Objectives: Rheumatoid arthritis (RA) is a worldwide problem for the medical field. When we scrutinize the disease Amavata in the Ayurvedic texts, it was first described as an independent disease in Madhava Nidana. It is a disease of Madhyama RogaMarga as it affects Sandhis and Hridaya Marma (Rasavahasrotomoola). Most of the symptoms of Amavata are related to the dushti of Rasavahasrotas like that of Aruchi, Agnimandya, Jwara,Angamarda, Guruta, Shoonata (shotha), etc. With this background the study was planned to study in detail about the nidanapanchakatva in amavata with special reference to rheumatoid arthritis. The study was also planned with a clinical method by administering Amavatari Rasa to study its effect on Amavata.
Materials and Methods: The conceptual part was done by referring the classical textbooks of Ayurveda as well as the text books of allied science. In the clinical part, 50 patients were selected for the study from O.P.D. and I.P.D. of Major S.D. Singh P.G. Ayurvedic Medical College and Hospital, Farrukhabad, U.P. and were prescribed Amavatari rasa 250 mg twice daily with luke warm water. Among 50 cases, 45 patients completed the treatment.
Results: Results were discussed in the form of conceptual and clinical aspects. In the conceptual aspect, the features of amavata in terms of rasavahasrotas involvement were highlighted and in clinical aspect the role of Amavatari rasa in the management of amavata is discussed. There was improvement of 32.24% in Vrishchikadamshavatvedana, swelling 37.59%, Sandhi graham 53.49%, Sparsha asahishnuta 35.12%, Angamarda 30.77%, Koshthabaddhata60%, Nidra viparyaya 40%, Gaurava 31.82%, Alasya 35.46%, Apaka 34%, Aruchi39.91%, Bahu mootrata 30.86%, Hrudgraha 24.81% and Trushna 21.875%. Overall effect shows that 54% of the cases had mild improvement, 18% had moderate improvement and remaining were unchanged or <25% improvement.
