The aim of this study was to determine the number of medicines specified being meetly acclimated grounded on the cases renal function according to the guidelines. Most important drug related problem in case with CKD is drug crimes. Numerous drugs and their metabolites are excreted through renal route. Therefore, acceptable renal function is important to avoid toxin. The materials and methods of this is a prospective observational study on medicine cure adaption study which was grounded on the collection of data on the diurnal visit to department of medicine, HICU and nephrology department in tertiary care unit. The study involves the analysis of specifics and cure adaption needed for CKD cases and compared with standard guidelines (K/DOQI) The cases who meet the criteria were involved in this study. The result of this study is total of 1063 specified medicines for 74 cases, 76(7%) medicines need a dosing adaption in renal failure and 989 (93%) medicines do not need. From 76 medicines that need a dosing adaption, the cure of 31(40%) medicines were acclimated and 45 (60%) medicines were not. Medicines that need an adjustment are meropenem, amoxicillin clavulanate, piperacillin tazobactam, cefoperazone sulbactam, amikacin, ranitidine and rosuvastatin. The finding of this study showed that the errors in dose of medications in CKD patients were high. Many drugs needed to be adjusted depending on the patient’s kidney functions. A team work of physicians with a pharmacist or a clinical pharmacologist is necessary for the detection and prevention of dose related medication errors which will improve the therapeutic outcomes and reduced the toxicity and side effects in patients with CKD.
