Abstract: A study to evaluate the effect of upright positions versus breathing exercise on pain and labour outcome in primigravida mothers during first stage of labour in Safdarjung Hospital of Delhi was under taken by Ms. Meenakshi Saini, under the guidance of Dr. (Mrs.) Molly Babu, Assistant Professor and Dr. (Mrs.) Sharda Rastogi, Assistant Professor towards the fulfilment of the requirements for the award of degree of master of nursing at Rajkumari Amrit Kaur College of Nursing, University of Delhi during the year 2016-2017. Objectives of the study were to: 1. Assess and evaluate the level of pain among primigravida mothers before and after the administration of upright positions. 2. Assess and evaluate the level of pain among primigravida mothers before and after the administration of breathing exercise. 3. Assess and evaluate the labour outcome of primigravida mothers after the administrations of upright positions 4. Assess and evaluate the labour outcome of primigravidae mothers after the administration of breathing exercise. 5. To find relationship between level of pain and labour outcome among primigravida mothers after the administration of upright positions. 6. To find relationship between level of pain and labour outcome among primigravida mothers after the administration of breathing exercise. 7. Compare the level of pain among primigravida mothers after the administration of upright positions and breathing exercise. 8. Compare the labour outcome among primigravida mothers after the administration of upright positions and breathing exercise. The conceptual framework of the study was based on Ernestine Wiedenbach helping art model. An experimental research approach with pretest posttest control group design with repeated measures was adopted. The population comprised of primigravida mothers during first stage of labour in Safdarjung Hospital of Delhi. The independent variables were upright positions and breathing exercise and dependent variables were labour pain and labour outcome among primigravida mothers during first stage of labour. The tools used were Structured interview schedule to collect the demographic data, structured assessment tool for selection of samples, Visual analogue scale to assess the level of pain, structured observation schedule for labour outcome at 4-6cm dilatation, 7-9cm dilatation, partograph and structured performa for assessing labour outcome after delivery. The reliability coefficient was established for structured observational schedule and structured performa by inter-observer reliability. The pilot study was conducted and was found to be reliable. The final study was conducted in labour room of Safdarjung Hospital, New Delhi. Data were collected from 10 Oct to 22 Oct 2016 for pilot study (10 in each experimental group 1, experimental group 2 and control group) and from 17 Dec 2016 to 7 Jan 2017 for final study (30 in experimental group 1, experimental 2 and control group). Purposive sampling technique were used to select the samples. The data were analysed using both descriptive and inferential statistics according to the objectives and hypothesis of the study. Major findings of the study revealed that the sample characteristic of both experimental 1, 2 and control group were nearly same. There was significant difference in the mean pre-test and post- test pain scores in both the experimental group 1 and experimental group 2. There was significant difference in post-test pain scores between experimental group 1 and control group. There was no significant difference in post-test pain scores between experimental group 2 and control group. There was significant difference in mean post-test labour outcome scores of primigravida mothers at 4-6 cm and 7-9cm dilatation and after delivery in experimental group 1. There was significant difference in mean post-test labour outcome scores of primigravida mothers at 4-6 cm dilatation but no significant difference at 7-9cm dilatation and after delivery in experimental group 2. Both the interventions (upright positions and breathing exercise) were equally effective in terms of pain at 4-6 cm dilatation but upright positions was more effective than breathing exercise at 7-9cm dilatation. Upright positions were more effective than breathing exercise in terms of labour outcome at 7-9 cm dilatation. Based on the findings and on the experience gained during the study following recommendations were made: A similar study can be conducted to assess the effectiveness of upright positions on labour outcome and labor pain among parturient mothers in II stage of labor. A comparative study can be done on various other positions to assess the labour outcome and labor pain among parturient mothers in I stage of labor. The same study can be done on different settings.
