Assessment of prescription pattern of anti hypertensive agent in patient with Hypertension and type 2 diabetes mellitus at tertiary care hospital
MATERIALS AND METHOD
In this study, a sample size of 137 patients prescription details are checked and the current prescription practice of clinicians and is compared with existing guidelines.Patient satisfying the inclusion criteria were indentifie department in general medicine.A Prospective and Observational study will be conducted in the department of cardiology and medicine. Data was collected by using a self-designed data collection form, which consists of details like patient demographic details like age, sex, weight, date of admission and date of discharge of the patients will be obtained. Drug related data such as name of drugs, doses, route of administration, duration & other laboratory data was be collected and documented in a suitable designed data collection form. The follow - ups will also be documented up to discharge.
RESULTS
A total of 22 adverse drug reaction were observed on treatment with different class of antihypertensive drug in 137 patient. Incidance of adverse drug reaction was found to be higher in patient more than 40 years in age. Diuretics were found to be the most frequently associated drug with adverse drug reaction (n=13), followed by beta blocker (n=6), Alpha blocker (n=1), ARBs (n=1) and Angiotensin converting enzyme (n=1). Most of the Adverse drug reaction associated with Electrolyte disbalance. Metformin and glimepiride were found to be most frequently associated drug with averse drug reaction 30% (n=3), followed by insulin 10% (n=1),Voglibos 10% (n=1). In the assessment of prescribing patterns of 137 patients,the number of patient on mono therapy was 73 (54%) and the patient on multiple drug therapy were 62. Among the patient receiving multi drug therapy the number patient on 2-drug combination therapy was 40 (33%). out of the 62 patient receiving multi drug therapy 14 (10%) was on 3-drug combination. Similarly the patient on 4-drug combination therapy was 3 (3%). Metformin and glimepiride were found to be most frequently associated drug with averse drug reaction 30% (n=3), followed by insulin 10% (n=1),Voglibos 10% (n=1).
CONCLUSION
The results of this study concluded that majority of diabetic Hypertensive patient were on mono therapy to control hypertension. Most of the patient were Calcium channel blocker (CCB),Beta blocker (BB) and ARBs. Patient were multiple drug therapy on ARBs/CCB and BB/CCB. The prescribing pattern was mostly consist with the evidence based guideline as majority of the patient were in control both diabetic and Hypertension.The above study Diuretic were found to be the most frequently associated drug with adverse drug reaction.The above study treatment with different class of anti diabetic drug Metformin and glimepiride were found to be most frequently associated drug with averse drug reaction, followed by insulin and Voglibos most of the ADR associated with Hypoglycemia.The above findings would be useful for physicians in rational prescribing.
