Abstract: Pharmacovigilance plays a critical role in detecting, assessing, understanding, and preventing adverse drug effects to optimize treatment outcomes and patient safety. Hypertension management increasingly relies on combination therapies, including angiotensin II receptor blockers (ARBs) and biguanide drugs like metformin, to achieve optimal control and reduce associated risks. Methods: A prospective observational study was conducted over six months to assess the real-world outcomes of ARB and biguanide combination therapy in hypertension and hyperglycemia management. Fifty patients with concurrent hypertension and diabetes were observed at a Super Specialty Hospital, capturing demographic data, medication use, and adverse events. Results: The study revealed various adverse drug reactions (ADRs) associated with combination therapy, including diarrhea (12%), dizziness (12%), rash/itching (12%), and polyuria (20%). Specific combinations, such as metformin + glimepiride + telmisartan, exhibited a 100% incidence of ADRs among prescribed patients. Conclusion: Comprehensive pharmacovigilance is crucial for optimizing treatment outcomes in patients with complex cardiovascular and metabolic conditions. This study provides valuable insights into the safety and efficacy of combined antihypertensive and antidiabetic therapies, emphasizing the need for vigilant monitoring and personalized treatment approaches to mitigate risks and enhance patient care.
