Abstract: The role of music in health care, particularly in critical care environments, aligns with the broader trend of incorporating non-pharmacological interventions to improve patient outcomes and experiences. A quasi-experimental was conducted to assess the Effectiveness of Nature Based Sound Therapy (NBST) on the Levels of Agitation and Physiological Stress among Patients with Mechanical Ventilator in MICU. The pre- test of experimental group, the levels of Agitation mean score was 2.8 and SD ±0.406, in post-test mean score was 4.8 and SD ±0.3, mean difference 2 and ‘t’ value was 9.75which is highly significant at P<0.001 level. In control group the pre-test levels of Agitation mean score was 2.8 and SD ±0.73, in post-test mean score was 4.8 and SD ±0.69, mean difference 2 and ‘t’ value 0.073 was not significant at P<0.001 level. This study showed that changing surrounding environmental sounds decreases environmental stimulation. Physiological parameters were reduce significantly after the completion of NBST intervention as a complementary adjunct is safe and can be applied by clinical nurse
