The accelerating digitization of healthcare records demands robust mechanisms to safeguard patient privacy and ensure data integrity. This paper presents a hybrid EHR protection framework that combines AES-GCM authenticated symmetric encryption with a permissioned Hyperledger Fabric ledger to deliver confidentiality, tamper evidence, and auditable access controls. Individual medical documents are encrypted and stored off-chain; compact SHA-256 digests of the ciphertexts are anchored on Fabric to provide immutable integrity proofs. A hierarchical key-wrapping scheme enforces fine-grained, role-based decryption privileges so that patient keys are released only to authorized parties, while comprehensive audit logs capture every upload, access, key operation, and verification event to enable future machine-learning–based anomaly detection. We implemented a prototype using a React frontend, Node.js cryptographic microservices, and a Fabric test network; experimental evaluation on representative PDFs shows low cryptographic overhead (encryption/decryption: ~8–12 ms) and predictable ledger commit times (150–180 ms) while accurately detecting tampering. The proposed design offers a practical, scalable approach for protecting clinical data and lays the groundwork for integrating intelligent monitoring to detect insider misuse and anomalous access patterns.
