Background: Breast cancer, the leading neoplasm among Indian women (26.6% prevalence, 192,020 cases in 2022 per WHO), frequently requires mastectomy for stages I-III treatment. This surgery commonly results in upper extremity impairments including pain, reduced shoulder range of motion, muscle weakness, lymphedema, and limitations in activities of daily living that persist beyond 3 months and compromise quality of life.
Purpose: To determine the prevalence of upper extremity impairments in post 3 months mastectomy patients using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and assess quality of life using the SF-12 health survey.
Methods: An observational cross-sectional study conducted at Shalini Tai Meghe Hospital, Nagpur among 76 female breast cancer survivors aged 25-60 years (mean 45-46 years), 3+ months post-mastectomy (mean 10 months, range 3-24). DASH questionnaire (0-100 scale; ≥40=severe disability) measured upper limb function while SF-12 assessed physical (PCS) and mental (MCS) health components. Data analyzed using SPSS 26.0 descriptive statistics.
Results: DASH scores revealed 0% normal function, 4% mild difficulty, 13% moderate difficulty, and 83% severe disability. SF-12 scores indicated compromised physical health (PCS=40) and mental health (MCS=45), both below normative mean of 50. Severe impairments predominated across all post-operative intervals.
Conclusion: Alarmingly high prevalence (83%) of severe upper extremity disability post-mastectomy underscores urgent need for systematic physiotherapy screening, early rehabilitation protocols, and multidisciplinary care to mitigate long-term functional limitations and restore quality of life among breast cancer survivors.
